Some refills may require an office visit. A nurse will contact you if this is the case.

We will need to know:

  • Name of the patient

  • Date of birth of the patient

  • Phone number

  • Name of the medication

  • Dose and frequency of the medication

  • The provider who prescribed the medication

  • The name and location of the pharmacy
