Prescription
refills may be requested by calling us during office hours at 978-251-3159. Please
allow
24-48 hours for processing of your refill request.
We will need
to know the:
- name of the patient
- date of birth of the patient
- phone number
- name of the
medication
- dose and
frequency of the medication
- the provider who presribed the medication
- the name and location of the pharmacy
Check
with the pharmacy directly to inquire if your refill has been
received. Some refills may require an office visit; a nurse
will contact you
if this is the case. |
