Our responsibility is to keep accurate records and bill appropriately; your responsibility is to provide us with up to date information, pay copayments at the time of service, and know the benefits and limitations of your insurance plan. It is your responsibility to talk with your insurance carrier and verify that we are in-network providers with your plan.

Your insurance company requires that you bring your insurance card with you to each appointment and that you pay your copays, which are due at the time of service.

For contracted insurances plans, we will submit claims, otherwise, payment is expected at the time of your appointment. Mastercard and Visa credit cards are accepted. Payment for non-covered services is required at the time of service.
